What we’re checking for
Whether LDN is clinically appropriate for you — including screening for the one contraindication that matters most: current opioid use. If LDN isn’t right for you we’ll tell you clearly, and suggest an alternative path.

A clinician reviews your case
A licensed physician who understands LDN reviews your intake summary and your complete symptom picture. They prescribe a titration protocol specific to your presentation — not a one-size-fits-all starting dose.
What proper titration looks like
You start at 0.5mg and increase every two weeks — 1mg, 1.5mg, 2mg — until you reach your therapeutic dose between 1.5mg and 4.5mg. Most patients find their dose in 8–12 weeks. This is how LDN is supposed to work.

Days 5–10
Your medication ships to your door

Months 1–3
Monthly titration reviews
While you’re finding your dose, a clinician reviews your progress monthly. If you need a dose adjustment, a new prescription is sent to the pharmacy automatically. Your next shipment arrives at the new dose.
What to expect in this phase
Some patients notice vivid dreams or mild sleep disruption in the first two weeks — this is normal and typically resolves. Most patients begin noticing changes between weeks 6 and 12, once they’re approaching therapeutic dose.

Month 4 onwards
Stable — quarterly check-ins
Once you’ve found a dose that’s working, we move you to quarterly reviews. Your medication ships automatically every month. You only hear from us when it matters.
Why quarterly
LDN is a long-term maintenance medication — not a course with an end date. For a stable patient on a consistent dose, monthly clinical reviews aren’t necessary. Quarterly is appropriate, clinically standard, and reflected in your subscription cost.

Common questions
What if I’ve already tried LDN and it didn’t work?
The most common reason LDN fails is inadequate dosing. If you were prescribed 0.5mg and never moved beyond it, you haven’t experienced what therapeutic LDN actually does. Complete the intake and our clinicians will review your history before making any recommendation.
Can I use Naltra if I’m on other medications?
The intake covers your full medication list. The one absolute contraindication is current opioid use — LDN blocks opioid receptors and will cause withdrawal if taken concurrently. Most other medications are compatible but the clinician review will confirm this for your specific case.
Is LDN covered by insurance?
Compounded LDN is almost never covered because it’s prescribed off-label. We provide a superbill with every subscription for out-of-network reimbursement attempts — but most patients pay out of pocket. At $149/month Naltra is designed to be the most accessible specialist-grade LDN service available.
What states do you operate in?
All 50 states. Telehealth requirements vary by state — some require a synchronous video call before prescribing. Your intake will confirm what’s required in your state before you proceed.
Can I cancel anytime?
Yes. No minimum commitment, no cancellation fee. If you cancel, outstanding prescription refills at the pharmacy are stopped. You can resubscribe at any time and pick up where you left off.
